Interactive workshops with management and/or plant personnel, focusing on one or more operational streams. Sessions include stress-testing existing practices to strengthen robustness when operations go wrong.
Typically following the Human Reliability Profile assessment, Last Line Partners supports and steers the deployment of selected tools to address identified improvement areas and embed sustainable performance gains over the long term.
Two-day session to review the fundamentals of human factors, including main concepts (protection layers, swiss cheese, domino effect), organization impact (competency, MOC, knowledge management, contracting), design impact (automation, alarms, error-prone design), frontliners' behaviors (preparation, deviation normalization, adverse environment, communication, situation awareness), human errors and decision-making.
